Season 14's D4 items hunt gets a lot easier once you know where this quest is actually pushing you. "Across the Threshold" is all about tracking down a Realmwalker, and if you have been waiting around for one to pop up on its own, you may be there a while. There is a random spawn route, sure, but there is also a cleaner path that most players end up using.
How the Realmwalker appears
You can get a Realmwalker in two ways. One is pure chance: finish a Surging Rupture with Mastery and hope the boss shows up. The other is the one people usually prefer. Head to the Realmwalker world event southeast of Zarbinzet. It does not run all the time, so you will want to watch your map for the Whisper marker that signals the event window. If the icon is there, move fast. These things do not hang around.
What you need to do in the event
Once the event starts, the game asks you to keep three Ruptures open for a short time. You will see a check mark on the minimap after each one is done. If you are solo, it is still manageable, just a bit messy. In a group, it is much smoother. Split up if you can. That way, each Rupture gets handled without wasting time running back and forth.

Be ready for the boss fight
When the three Ruptures are done, the Realmwalker steps into the center of the arena. This fight is not locked behind a high difficulty setting, so do not overthink it. Normal mode is fine if you just want to finish the quest. The real thing to watch for is the three pillars that appear during the battle. Take them out quickly. If you leave them alone, they can turn into Realmbreakers, and that only makes the fight clunkier than it needs to be.
Finish the last stretch and talk to Dannica
After the Realmwalker goes down, a portal opens to the Deathtoll Chamber. Go through it. The layout feels a lot like another Rupture, but the job is different this time. Kill enemies, build out the ritual circle, and keep moving until the final objective clears. Then head back and speak with Dannica. If you want the cleanest route, follow the world event, keep the Ruptures under control, and buy D4 Gold only after you have wrapped up the quest and are ready to gear up for whatever comes next.
